Summer Staff Paperwork

Congratulations – you have been hired! Now, time to get to work:

Read and Remember:

The following items are things that you should be sure to read and remember:

Summer Staff 2013 Handbook – This will answer a lot of questions you have, including “When do I get paid?” and “How can friends send me letters?”  The handbook also covers specific expectations and rules.  You will be held accountable for this information.

Covenant Affirmations – This link tells you more about the Covenant denomination, which owns Covenant Harbor.

New Staff Survival Guide – First year staff members will find this very helpful!

Download and Return to Youth Ministry Team:

The following documents are things that need to be downloaded, filled out, and returned to the Youth Ministry Team via mail or email:

Summer Staff 2013 Bio Form – Please fill out the staff bio and send it to us.

Summer Staff 2013 Health History Consent Release Form – This form must be filled out completely and sent with a copy of your insurance card if you will not have it with you during the summer.

Summer Staff 2013 Skills Inventory – Fill out this form to let us know what skill(s) you’re interested in leading.

Electronic Communication Agreement – All employees must agree to this policy.  Read, sign and return.

Financial Paperwork – these forms must be correctly filled out to get paid, so read instructions carefully.  Upon arrival, first year staff need to present a passport or driver’s license and social security card.

W-4 (now available!)

          I9 Form

Direct Deposit – We require paychecks to be directly deposited in your bank account.  Fill this out and bring a voided check with you to camp.

New Hire Reporting - Fill out only the top employee section.

Parental Consent Form (for all staff under age 18) – Discuss this form with your parents.

Pray!

Also, please continue to prepare for the summer with prayer – it is the only adequate way to prepare for a demanding summer.  Commit the summer to the Lord by praying for the team that will serve together and for the campers and parents coming to camp.  Know that we will be praying for you as you prepare to join the team of Summer Staff 2013 at Covenant Harbor!
